Insulated roof build-up over new blockwork

This is the detail that decides whether a new build or extension performs: insulation boards laid continuously over the top of the structure, with the rafters set out above them.
Running the insulation over the top rather than only between the timbers means the rafters themselves aren't cold bridges through the roof. The foil-faced boards are butted and taped, and the timbers are laid out square across the whole plane before any covering goes near it.
It costs slightly more in timber and takes slightly longer. It's also the difference between a roof that performs as specified and one that performs as built — which is the whole argument for low-energy building being a workmanship question rather than a shopping question.
